Was there any vibration?
Tail housing of the transmission has been destroyed, correct?
My first guess would be the driveline exploded and took everything out, it was a one piece, correct?
How far out of the tailshaft was the yoke?
Since it looks like most the transmission fluid is on the ground, that is why my first guess is the driveline, we need to figure this out, don't want it to happen again to anyone!!!

When you get it apart, you need to check that area in the transmission, if the shafts and gears are still intact, then I would say driveline.
I don't see any gears or shafts that hold gears in the parts in the bed, all I see is aluminum alloy and the shifter mechanism which is held on by the tailshaft housing.
So far I say driveline, as what happened is what I expect would happen if the driveline let lose, total carnage, your lucky shit didn't come threw the floor boards.
Were you floored or shifting gears when it happened?
Was there a vibration at any speed?
Did the driveline have any dents in it?
Was the driveline balanced?
How far was the yoke sticking out of the tailshaft?
Is the shaft that the yoke slides onto still there pointing backwards?
I cannot tell from the video how far the yoke was sticking out of the tailshaft because it was not connected to a driveline.

Driveline was balanced.
The yoke in the video isn't the yoke I used. I used the 720 yoke. But there wasn't any vibrations, I shifted into 5th 65 is what my phone speedO said,was there maybe a minute or a little less. Then Bang! Huge noise, think I lifted off the ground another bang... rears must have locked up or something and that sent me into a spin I think. .

In my opinion mind you, if all the shafts/gears in the transmission are not broken, and there was no warning, meaning no noise or vibrations, and the rearend is good, meaning that it had fluid in it, said fluid is a proper color, and since you said you pushed it off the road it's not locked up, I would call driveline.
When the driveline lets loose, a LOT of shit hits the fan, it becomes very unbalanced, and when you have that much weight out of balance/control while it's turning, shit starts moving around weather it wants to or not, it can turn/flip a vehicle over if the driveline jams into the ground.
You said that it shifted the transmission sideways far enough for the fan to destroy the radiator, well that is likely the force of the driveline pulling the back of the transmission sideways when it hit something, that would have likely destroyed the tailshaft of the transmission at the same time, it could have been the driveline biting into the ground that sent the truck into a spin, which is more sideways motion exerted onto the tailshaft.
I see what looks to be my tranny mount is still in position, but the rubber part of the tranny mount is missing, the tranny was ripped off the mount.
I suppose that something in the transmission could have been bad, as that bushing had worked it's way backwards for some reason, but when an engine comes apart, there is usually only one hole in the side of the block, and it doesn't tear the bottom of the block off, but a driveline swinging around.........
If the rear u-joint let loose at speed, it likely would have had the same result, total carnage, but they would usually give some kind of warning, vibrating, clunking, knocking, I am very surprised that there was no vibration/noise before the shit started flying apart.
I have at least one 520 driveline out back, but I am not a fan of the one piece driveline, and this is why, I used to have a 47 Chevy pickup(longbed), I had a one piece driveline made for it, when I went to pick the said driveline up from the shop, it was huge and very heavy, it was 4 inches round, I asked about that right away, they told me that the longer it is, the bigger round it had to be, some formula them guys go by, now the reason I brought this up is that the 520 only had a J13 in it, and in some other countries it might have had the J15/J16, an LZ23 might put a little more force onto that skinny little driveline than the J series engine can, just something to think about.

Jeff Hino (Icehouse) had something similar happen to him once. total fucking carnage like that. He was on I5 too. Transmission exploded like that due to the driveline failre & spinning around that fast IIRC
